Integrated Wellness (“iWellness,” “we,” “us,” or “our”) respects your privacy. This Privacy Policy explains what information we collect through iwellnessaz.com and our related marketing (including social media lead forms), how we use and share it, and the choices you have. This policy applies to our website and marketing activities; it does not cover the confidentiality of your protected health information (PHI) as a patient, which is governed separately by our HIPAA Notice of Privacy Practices, available at our office or upon request.

We collect information in the following ways:

Information you provide directly

• Contact details: name, email address, phone number

• Appointment and inquiry details: the service(s) you're interested in, preferred contact time, and any message you send us through a contact form, lead form, or by phone

• Newsletter sign-up information (email address)

• Payment information you submit through our online payment portal (processed by our third-party payment provider — we do not store full payment card numbers on our own servers)

Information collected automatically

• Device and usage data such as IP address, browser type, pages viewed, and referring website, collected through cookies and similar technologies

• Interaction data from social media advertising, including Meta (Facebook/Instagram) Lead Ads forms, when you submit a form through one of our ads

2. How We Use Your Information

We use the information we collect to:

• Respond to your inquiry and schedule or manage appointments

• Provide the services you request and communicate with you about them

• Send appointment reminders, billing information, and administrative updates

• Send our newsletter or promotional communications, if you opted in

3. How We Share Your Information

We do not sell your personal information. We share information only in the following circumstances:

• Service providers: with vendors who perform services on our behalf, such as our scheduling platform (Zocdoc), payment processor (MyAestheticsPro), email/newsletter provider, and IT/hosting providers (Wix). These providers are only permitted to use your information to perform services for us.

• Advertising platforms: when you submit a Meta (Facebook/Instagram) Lead Ad form, your responses are shared with us by Meta so we can follow up with you. See Section 4 for more on advertising and analytics.

• Legal and safety reasons: if required by law, subpoena, or other legal process, or to protect the rights, property, or safety of iWellness, our patients, or others.

• Business transfers: in connection with a merger, acquisition, or sale of assets, subject to standard confidentiality protections.

4. Advertising, Cookies, and Meta Lead Forms

Our website and social media pages may use cookies, pixels, and similar tracking technologies (including the Meta Pixel) to measure ad performance and show relevant ads. When you submit a lead form on Facebook or Instagram, Meta shares the information you provide with us so our team can contact you; that submission is also subject to Meta's own Data Policy.

You can control cookies through your browser settings, and you can manage the ads you see and the information Meta uses through your Meta Accounts Center ad preferences.

5. Your Choices

• Opt out of marketing emails or texts at any time using the unsubscribe link, by replying STOP, or by contacting us directly

• Request access to, correction of, or deletion of the personal information we hold about you, subject to legal and recordkeeping requirements (some information, such as medical/billing records, must be retained under law)

• Arizona and other state residents may have additional rights under applicable state privacy laws; contact us using the information below to make a request

6. Data Security

We use reasonable administrative, technical, and physical safeguards to protect the information we collect. No method of transmission or storage is completely secure, and we cannot guarantee absolute security.

7. Children's Privacy

Our website and services are intended for adults. We do not knowingly collect personal information from children under 13. If you believe a child has provided us information, contact us and we will delete it.

8. Third-Party Links

Our website may link to third-party sites (such as our scheduling and payment portals). This Privacy Policy does not apply to those third-party sites, and we encourage you to review their privacy policies.
